                    Pros And Cons Of Spinal Decompression Treatment

                    Spinal decompression is a high-tech form of traction used to treat disc bulging and degeneration and their associated pain and disability.

                    Spinal decompression differs from regular traction in that it uses computer-controlled motors to trick the spinal muscles into remaining relaxed and uncontracted during the treatment session. This results in much greater reduction in disc pressure than regular traction, which usually must work against the resistance of contracted muscles. Most people get excellent results from spinal decompression treatment, but some do not get any improvement, and a few may actually get worse. The following is an explanation of the reasons for the failures in spinal decompression treatment and ways you can know whether or not spinal decompression is likely to help in your particular circumstances.

                    I have been using spinal decompression in San Antonio for a few years now, and while it is a highly effective treatment for many people, it is not for everyone. Unfortunately, the costs of providing spinal decompression services sometimes influences doctors to recommend it in cases where it may not be entirely appropriate. Even with proper patient selection, spinal decompression is not 100% effective – no treatment is. But when doctors are not as discriminating as they need to be in what patients they accept, the results can be far from ideal and some patients may even get worse with this form of treatment when doctors prescribe it inappropriately for financial reasons.

                    From my experiences in my San Antonio practice, the patients who get the best results with spinal decompression are those with one or more bulging/herniated discs and patients with mild to moderate disc degeneration. Patients who have previously undergone disc surgery are somewhat more complicated, but most still get good results when they meet the selection criteria (no metal implantation in the spine, no spinal instability, and no impaired healing at the site of surgery). The vast majority of people who meet these criteria get very good results and are usually able to return to their normal activities without any significant pain.

                     

                    While there are case reports that indicate complete disc ruptures can be treated with spinal decompression, in my experience, people with actual ruptures tend to not do very well with this form of treatment. It should be noted that even doctors sometimes incorrectly call a disc bulge or herniation a rupture. True disc ruptures (also called extrusions and sequestered discs), in which the inner gel of the disc is actually leaking out, are relatively rare, so it is good to find out for sure what your actual condition is.

                    Besides the obvious importance of only using spinal decompression in cases where it is really appropriate, it is very important for the machine operator to be well-trained to ensure proper set-up of the machine for the patient. There are several different decompression machines on the market, each with their own specific set-up procedures. Whatever machine is being used, it is critical to have ongoing refresher training for the operating technicians to be sure that all patients are being set up on the machine exactly right. While the better machines are unlikely to produce any major injury if they are not set up exactly right, results can suffer tremendously if the patient is not positioned and secured on the machine correctly on every visit. I highly suggest asking any spinal decompression provider how frequently the technicians undergo training and only work with those providers who do re-training at a minimum of every 3 months.

                    One other consideration is the fact that some patients are not good candidates for spinal decompression treatment because of their inability or unwillingness to follow the recommended treatment protocol. In my experience, the primary reasons why a patient can’t or won’t follow the treatment recommendations are usually related to money and/or time.

                    A complete spinal decompression program of care that inclues decompression as well as additional treatments may seem expensive to some people. Since spinal decompression is an alternative to disc surgery though, a cost comparison with surgery will show spinal decompression to be much less expensive with statistically much better resuts, making it a good value in most cases. Even so, some people try to lower the cost by attempting to reduce the amount of treatment they get. In my experiences from my San Antonio office, this approach is often a very big mistake. The people who discontinue care prematurely will often feel better and return to activities before their disc condition is stable. This can lead to re-injury and even worse problems. In my opinion, it is far better to invest in the recommended treatment in order to get maximum benefits.

                    Of course, some people fail to follow treatment recommendations primarily because they have busy lives and are unwilling to spend the time on getting better. This type of patient tends to miss a lot of appointments and may go long periods of time between treatment sessions. Unfortunately, the success of spinal decompression usually depends on getting the recommended amount of treatment at the recommended frequency of treatments. The effectiveness and results are not as good when people don’t make time for their appointments. What some busy people fail to realize is that if they don’t set aside time to properly deal with their health problem, sooner or later that health problem will deteriorate to the point where it forces them to make time to get treatment, and this often occurs when they are at their busiest and treatment is least convenient. It is my recommendation to invest the necessary time in getting better, rather than do the treatment intermittently and not get the best results.

                    To summarize, spinal decompression can be an extremely effective treatment for people suffering with pain from bulging and degenerated discs, but it is important that doctors select patients for this treatment carefully and make sure that the technicians operating the equipment get refresher training regularly to be sure patients are being set up on the machine properly. Patients need to take the responsibility to follow the treatment recommendations to allow for maximum correction and healing.

                      Stenosis of the spine can occur in different areas of your spine and produce different symptoms. But basically all types result from a narrowing of the spinal canal–most often the result of arthritis.                       The vertebrae are a series of bones connected to each other forming the neck, also known as the cervical spine. The spinal canal, which encloses the spinal cord, runs through the vertebrae. The spinal cord contains major nerves that allow arm and leg movements, sensation, including bladder control and bowel movements.

                      Cervical stenosis occurs with age as the intervertebral discs starts to lack water content and hardens. The discs can shrink in height and stick out into the spinal canal. Spinal joints also bulge and protrude into the spinal canal. When the spinal canal narrows, the resulting pressure on the spinal cord leads to another condition called cervical myelopathy, which affects nerve functions.

                      Cervical Stenosis Symptoms

                      Symptoms of cervical stenosis usually are imperceptible at first. If the stenosis has advanced to cervical myelopathy, the patient may experience neck and arm pain, weakness, and difficulty in moving the arms and legs. Incontinence also occurs in later stages of the disease. Symptoms may appear gradually or develop rapidly.

                      Early detection plays a crucial role in the prevention and treatment of cervical stenosis and cervical myelopathy. Your doctor will perform a physical examination and diagnostic tests and recommend an MRI (Magnetic Resonance Imaging ) and CT (Computed Tomography) scan to be able to see the level of narrowing of the spinal canal. You may have to undergo other tests for a complete diagnosis.

                      Treatments for Cervical Stenosis

                      Depending on the stage of cervical stenosis, treatments may be operative or  non-operative . Usually, patients who have severe fragility and pain in the affected areas and difficulty in walking require surgery. Non-operative or conservative treatment, which includes cervical stenosis physical therapy, is ideal for mild cases.

                      Patients should understand that  cervical stenosis physical therapy  would not reduce the narrowing of the spinal canal or bring it back to normal size. The goal of cervical stenosis physical therapy is long-term pain management and increased function that will enable the patient to control pain effectively and function normally without having to undergo surgery.

                      Cervical stenosis physical therapy starts with improving flexibility in the neck, arms and legs through stretching exercises. It is also important to increase circulation and develop endurance in the arms and legs with cardiovascular exercises such as swimming and treadmill exercises. Your therapist may also add strengthening exercises in your program. While most of these exercises are always under professional supervision, your therapist will also provide you with exercises that you can perform independently.

                      Supervised cervical stenosis physical therapy may take three or more months. If your condition does not improve after cervical stenosis physical therapy, your physician will then recommend surgery.

                            Bulging disk, also known as a Contained Spinal Disorder, is a condition that occurs usually in spinal or lumbar region, lower back and in few cases witnessing with neck region. It commonly occurs as the human body ages and with the increase in age degeneration of inter vertebral disk occurs. Disks are soft material that engulfs the vertebrae of spinal chord and provides a cushioning support. When the disc moves out of its normal location or radius bulging disk occurs.

                            To understand the phenomenon of bulging disk, we have to understand the anatomy and physiology of intervertebral disc.

                            Intervertebral Disc
                            Spinal chord is encased by spine made up of intervertebral discs whose primary function is supporting the body weight. The repeated daily stress over time results in degeneration of the disc. Each vertebra of a spine contains intervertebral disc. The stress of every day movements is transferred to these discs. They act as shock absorbers to absorb any kind of impact and provide a cushioning support.

                            A healthy intervertebral disc contains a great amount of water in its center known as nucleus pulposus. This water content provides a spongy behavior to disc.

                            An excessive pressure or injury can be harmful for the structure of outer ring of ligament covering nucleus pulposus. This ligament is better known as annulus.

                            Pathology and Mechanism
                            Because of aging factor the scars received by annulus over time lead to small cracks to this ligament and make it weaker. Eventually this leads harm to nucleus pulposus and it starts loosing its water content. Due to the loss of water content, intervertebral disc starts loosing its spongy nature and degeneration occurs. This leads to further development of cracks due to increased impact and more loss of water. So the cushion effect diminishes and spine can’t absorb stress any more.

                            The space between two intervertebral discs decreases, even collapses which results in the movement of two adjoining disc to come closer. Because of this movement, there is a considerable shift in location of facet joints at the back of spine. Bone spurs by activities of osteophytes which began to accumulate around the disc spaces and facet joints. This spurring process is a natural response by spine to reduce the excessive motion of that typical segment of spine.

                            Pathologically, this condition is known as Bulging Disc. When this bones spurs start growing towards spinal canal, exerting and protruding pressure on spinal nerve, the condition become much more painful. This condition is also known as spinal stenosis.

                            Symptoms
                            In case of bulging disc, the nature of the symptoms usually depends upon the precise location of the affected intervertebral disc in the spine. Person may suffer pain and discomfort in their legs and hips if disc at lower back is affected while others may suffer such pain in arms if the intervertebral disc is located near shoulder region in upper back. The symptoms from this degenerating disc include discogenic pain.

                            Diagnosis
                            Magnetic resonance imaging (MRI), CT scans and X-rays can confirm the presence of bulging disk. The diagnosis is made only after getting or identifying the complete case history by the physicians. Here X-rays can be used to confirm that the intervertebral disc space has been decrease. MRI tests are carried out to decide a loss of water content in the intervertebral disc.

                            Treatment
                            The treatment of bulging disc can be of two types.

                            • a) Conservative Treatment- According to the assessment of seriousness and severity of the pain treatment is decided. Some problems need an immediate medical attention while others do require a plenty of rest with a supplement of anti inflammatory, analgesic pain killers like cortisones. In some cases, if the protrusion of disc is much more in the spinal canal then the surgery is required. The treatment is aimed at making the patient comfortable as quickly as possible, getting him/her back to normal state in a time bound carefully designed rehabitation program to reduce further wear and tear of the disc.
                            • b) Rest - Rest plays the most important role in the treatment of bulging disc. In most of the cases, a complete, sound rest is what your body is craving for. It is used to reduce the stress and pressure off your spine. Sometimes a placement of pillow under knees can help a lot. Although, rest is most important parameter you should not get stucked with a complete bed rest and should try to return to normal activities gradually. Rest combined with a small amount of recommended exercises can do wonder.
                            • c) Physical Therapy – In most of the cases a physical therapy proves of immense help in rehabitation program. Workout with an expert physical therapist helps in reducing inflammation and pain, improving and rebuilding your strength, and returning back to normal activities with a greater ease. A well rounded physical therapy is aimed at controlling symptoms, practicing mobility and safe exercises. In case of patient suffering from back pain gradual physical exercises is the best way to combat the pain. It also teaches you to maintain a correct posture to offer maximum rest and less stress to spine.
                            • d) Epidural Steroid Injection (ESI) – They are used as pain killers. They are of great help to control inflammation.

                            A bulging disc though common in old age, it is not necessary that everyone will experience the pain associated with it. Only when intervertebral disc extents towards spinal canal, pain arises.
